We ask that parents please print out and complete the form at home and bring along the completed form to training. 

Registration Checklist

Please check and ensure the following has been supplied/completed:

  • This form has been read and all areas completed DOWNLOAD HERE
  • Both player and parent/guardian signatures are present
  • Full payment has been made/provided
  • A passport style photo has been supplied (as a jpeg)
  • If you are new to the club supply a proof of age such as passport or birth certificate (photocopy – no originals accepted)
  • You have read and understand the Club’s code of conduct, social media policy and the FA Respect literature.

As a volunteer-led organisation, we welcome and appreciate any sponsorship into the club as well as any help that can be provided.

Sponsorship of St Andrews Football Club is a great way of providing community support for the local children, and encouraging them to take part in a hobby which promotes respect, teamwork and physical fitness. It also helps promote your own business via various mediums such as our website and by displaying your company name and logo at our grounds.
